Student Agreement Form

Please sign and upload the Student Agreement Form

There are two parts to the application process.

First, you’ll need to download and sign the Level 5 Student Agreement Form Document. Once you’ve finished it, you’ll use the submission form further down this page to upload it to us.

Step 1 – Download your student agreement document

Important: The file will save directly to your computer’s Downloads folder. Once downloaded, follow the steps below:

1. Go to your Downloads folder and open the student agreement form.

2. Sign and Date the form.

3. Save your completed application using the file name: [Your Name] – Level 5 Student Agreement Form

4. Your completed form is now ready to upload in Step 2.

Step 2 - Submit your completed student agreement form

When your agreement document is ready, scroll down to the Submission Form below.

Complete your contact details and attach your signed Document when prompted.

Once you submit the form, your signed agreement will be uploaded to your record.
